Please note that all new referrals must contact office by phone to complete a confidential initial intake with office manager prior to consultation with me. Filling out the online contact form below may delay your referral process as I don't always receive these in a timely manner. The office manager will expedite your referral and she can provide information about my fees and availability for scheduling. All completed intakes will be followed up by a direct telephone contact with me before scheduling initial appointments.
